Submitted applications and materials will not be returned.
Continuing Education Applications may be submitted via mail, online, fax or email.
After receiving CE approval, if there is a change in course title, speaker(s), lab assistants, contact hours or agenda, you must submit the
CE Amendment Form along with the $15.00 fee. The fee may be paid with a check or credit card. Online payments are also accepted by clicking here
