Application fee for Individuals submitting a course, seminar or distance learning for CEU approval for PTs and PTAs licensed in California.
Please
allow 14 days for application review. The contact person will be
notified via
email, if provided. If no email address is provided, then notices will
be sent via
U.S. Postal Service.
Application
fee $35.00 per course. Submit only one course per application. Fees are
non-refundable.
Submitted applications and materials will not be returned.
RUSH Fee is an additional $25.00 and review will be completed within 48 hours.
